Instructions on filling out the registration form:

1. Please fill out the fillable form directly from this page or download and fill the form from your device.
2. Once you have completed the form, click the download button on the top right corner and select “with your changes”.
3. Save the completed form to your device.
4. Send us an email with the saved file as an attachment to info@entvancouver.ca

Please note: This form work best on computers. If you have trouble filling out the online form, please arrive 10 minutes early to your appointment to fill it out in person.
